Financial Affidavit For Income Over $50,000
This form should be used when you are involved in a family law case which requires a financial affidavit and your individual gross income is $50,000 or more. The affidavit must be completed using monthly income and expense amounts. If you are paid or your bills are due on a schedule which is not monthly, you must convert those amounts.

Financial Affidavit For Income Less Than $50,000
This form should be used when you are involved in a family law case which requires a financial affidavit and your individual gross income is UNDER $50,000 per year. The affidavit must be completed using monthly income and expense amounts. If you are paid or your bills are due on a schedule which is not monthly, you must convert those amounts.

Parenting Class
Both parties are court ordered to take a parenting class when there are minor children born of the marriage. Upon completion of the class, please forward the certificate of completion to my office so that I can file it with the Clerk of the Court. Please review this list for information of court approved providers.

Parenting Plan Form 12.955(a)
A Parenting Plan is required in all cases involving time-sharing with minor child(ren), even when time-sharing is not in dispute. The Parenting Plan must be developed and agreed to by the parents and approved by the court. If the parties cannot agree to a Parenting Plan or if the parents agreed to a plan that is not approved by the court, a Parenting Plan will be established by the court with or without the use of parenting plan recommendations.

Relocation/Long Distance Parenting Plan Form 12.995(c)
This form or a similar form should be used in the development of a Parenting Plan when you are planning to relocate your or the child(ren)’s principal residence more than 50 miles from the principal place of residence at the time of the last order either establishing or modifying time-sharing, or at the time of filing the pending action to either establish or modify time-sharing.
